License No. LP2653 CONSENT ORDER
IT IS HEREBY STIPULATED AND AGREED by Renee Fredrickson Ph.D., LP. (Licensee)and the Minnesota Board of Psychology (Board) as follows:
1. During all times herein, Licensee has been and now is subject to the jurisdiction of the Board from which she holds a license to practice psychology in the State of Minnesota.
2. On May 7, 1999, Licensee entered into a Stipulation and Consent Order with the Board. A copy of that document is attached hereto and incorporated by reference in its entirety as Exhibit A.
FACTS
3. For the purpose of this stipulation, the Board may consider the following facts as true:
a. On May 7, 1999, the Board adopted a Stipulation and Consent Order (S&CO) that restricts and conditions Licensee's license to practice psychology in Minnesota. This Order permanently restricts Licensee from providing therapy to clients whose therapy issues involve cult, ritual, or satanic abuse. Licensee is also permanently restricted from providing supervision or consultation to other therapists with respect to cult, ritual or satanic abuse.
b. The May 7, 1999. S&CO further restricts Licensee from providing hypnosis or guided imagery services to clients except under supervision or training, until she can demonstrate to the Complaint Resolution Committee's (Committee) satisfaction, through the appropriate combination of education, experience, training and supervision, that she is competent to provide these psychological services.
c. The May 7, 1999, S&CO also placed conditions on Licensee's license to practice psychology in Minnesota; one of which provided that:
1) Licensee shall complete a psychological evaluation and the Board shall receive the evaluator's report within 90 days of the date of this order. The evaluation shall be performed by a licensee of the Board who is not acquainted personally or professionally with Licensee and who has been approved in advance by the Board's Complaint Resolution Committee from a list of three names provided to the Committee by Licensee. The evaluator shall submit a report to the Board that includes and addresses the information and issues outlined on page 23 of the Order; Items 97.b.1 through 9.
2) Licensee's current therapist shall submit a report to the Board within 90 days of this Order. The Report shall include and address the information and issues outlined on page 24 of the Order, Items 97.c.1 through 5.
3) Based upon the evaluator's report and the current therapist's report or treatment record, the Board reserves the right to impose additional appropriate disciplinary action on Licensee's license.
4) If any treatment is recommended, by the evaluator or Licensee's current therapist or is required in the judgment of the Committee based on Licensee's treatment records, Licensee shall comply with the treatment recommendation. The treating therapist shall be a different person than the evaluator and shall be pre-approved by the Committee.
5) If treatment is recommended, the treating therapist shall submit a report to the Board every three months and at the time Licensee petitions to have the conditions removed from her license. Each report shall provide and address the information and issues outlined in Items 97.f.1 through 8 on page 25 of the Order.
d. On July 26, 1999, the Committee approved an independent psychologist to complete the psychological evaluation of Licensee as required by the May 7, 1999, S&CO. On July 30, 1999, Licensee met with the evaluating psychologist.
e. On September 3, 1999, the evaluating psychologist submitted his psychological evaluation of Licensee to the Board. He recommends that Licensee's practice be supervised by a licensed psychologist approved by the Board; that Licensee be provided treatment by a licensed psychologist approved by the Board; that both the supervisor and treating therapist submit reports to the Board either quarterly or twice yearly; that the supervisor and the therapist not be the same person; and that appropriate releases be executed allowing the exchange of information between them. He also recommends that the Board provide Licensee with the names of a licensed psychologist it approves to serve as her supervisor and those it approves to serve as her therapist rather than having Licensee submit names to the Board for approval. Finally, the evaluating psychologist recommends that the supervisory structure be in place for an extended time, suggesting monthly supervision for a two year period as a reasonable initial estimate and that the treating therapist provide therapy that is not short term but an ongoing one of some duration.
f. On September 17, 1999, the Committee recommends that the full Board adopt the following recommendations that are in keeping with the evaluator's report.
Based on the foregoing, the Board issues the following:
ORDER
4. Upon this stipulation and record and without any further notice or proceedings, the Board hereby AMENDS the May 7, 1999 Stipulation and Consent Order in this matter as follows:
Paragraph 97 is amended to add the following subparts:
o. Supervision. Licensee shall practice psychology only under the supervision of a licensed psychologist whose name shall be provided to her by the Board's Complaint Resolution Committee. Supervision shall commence within 30 days of the date of this Amended Order. Licensee shall meet with the supervisor every two weeks for a minimum of one hour per meeting to discuss the nature and content of Licensee's practice, focusing particularly on the issues raised in the evaluating psychologist's report of September 3, 1999. Every client record shall be subject to the supervisor's review. The supervisor shall determine which of the Licensee's cases are reviewed during each supervisory meeting, by a random sampling or other method determined by the supervisor. The supervisor shall have the discretion to periodically consult with Licensee's treating therapist regarding overlapping areas of responsibility.
p. Supervisor's Reports. The supervising psychologist shall provide a report to the Board one month from the date of the first supervisory meeting. Subsequent reports will be provided to the Board every three months, thereafter, and at the time Licensee petitions to remove the restriction and conditions removed from her license. All reports except the first shall be submitted on the first day of the month in which they are due. Each report, except where stated otherwise, shall provide and/or address:
1) In the first report, verification that the Licensees supervisor has received and reviewed a copy of the May 7, 1999 Stipulation and Consent Order, the evaluating psychologist's report of Licensee dated September 3, 1999, and this Amended Stipulation and Consent Order;
2) The dates on which supervision took place with Licensee; 3) The method by which supervision was conducted; 4) A detailed discussion of the issues addressed at each supervisory meeting;
5) Licensee's progress in maintaining appropriate professional boundaries with her clients;
6) The supervisor's opinion as to Licensee's progress in providing fit, competent, and ethical psychological services;
7) Any other information that the supervisor believes would assist the Board in its ultimate review of this matter;
8) At the time Licensee petitions for removal of the restriction and conditions on her license, the supervisor's report shall include an assessment of Licensee's ability to conduct herself in a fit, competent, and ethical manner in the practice of psychology.
q. Licensee's Reports. Licensee shall submit a report to the Board every three months and at the time she petitions to have the restriction and conditions removed from her license. All reports shall be submitted on the first day of the month in which they are due. Each report shall provide and address:
1) The dates on which the supervision took place;
2) A detailed discussion of the issues addressed at each supervisory meeting.
3) A detailed discussion of the knowledge Licensee has obtained from her meetings with the supervising psychologist, including Licensee's own statement as to her comprehension and knowledge of ethical issues encountered in her practice;
4) A detailed discussion in which Licensee applies the knowledge she has obtained to the specific violations listed in the Regulations and Facts sections of the May 7, 1999 Stipulation and Consent Order;
5) A detailed discussion regarding the specific procedures Licensee will implement in her practice to apply the knowledge she has obtained from the meetings with the supervising psychologist;
6) Any other information that Licensee believes would assist the Board in its ultimate review of this matter; and
7) At the time Licensee petitions the Board to remove the conditions and restrictions from her license, Licensee shall include in her petition, a discussion of her reasons for believing that she is capable of conducting herself in a fit, competent, and ethical manner in the practice of psychology.
r. Treating therapist. Licensee shall obtain therapy from a licensed psychologist whose name has been provided to her by the Board's Complaint Resolution Committee. The therapy shall focus particularly on the issues raised in the evaluating psychologist's report of September 3, 1999. Licensee shall commence her therapy within 30 days of the date of this Amended Stipulation and Consent Order. The frequency and duration of the therapy shall be determined by the treating therapist. The therapist shall have the discretion to periodically consult with Licensee's supervisor regarding overlapping areas of responsibility.
s. Treating Therapist's Reports. The therapist shall submit a report to the Board one month from the date of the first therapy session. Subsequent reports shall be provided to the Board every three months thereafter and at the time Licensee petitions to have the restriction and conditions removed from her license. All reports except the first shall be submitted the first day of the month in which they are due. Each report, except where indicated otherwise, shall provide and/or add ress the following:
1) In the first report, verification that the therapist has received and reviewed a copy of the May 7. 1999 Stipulation and Consent Order, the evaluating therapist's report of September 3, 1999 and the Amended Stipulation and Consent Order;
2) In the first report, identification of a treatment plan devised specifically for Licensee. Any subsequent changes made in the treatment plan are to be identified in later reports;
3) A statement of the involvement between Licensee and the therapist, including dates, number, and frequency of meetings;
4) Licensee's therapeutic progress and compliance with the treatment plan;
5) The therapist's opinion as to Licensee's capacity to understand her professional role and the boundaries of that role and Licensee's ability to distinguish between her personal and professional needs, beliefs, identity, and behavior;
6) The therapist's opinion as to the need for continuing therapy or Licensee's discontinuance of therapy;
7) Any other information which the therapist believes would assist the Board in its ultimate review of this matter; and
8) At the time Licensee petitions for removal of the above referenced restriction and conditions, the therapist's report shall include an assessment of the Licensee's ability to conduct herself in a fit, competent and ethical manner in the practice of psychology.
The Board further reserves the right to impose additional appropriate disciplinary action on Licensee's license based on reports of Licensee's treating therapist if the Board determines that Licensee's mental health has deteriorated so as to pose a risk to the public health and welfare.
t. Releases. Licensee shall execute releases allowing her supervisor and treating therapist to communicate with each other. Licensee shall also execute releases allowing the Board to provide data related to this Amended Stipulation and Consent Order to her supervisor, treating therapist, and any other professional Licensee has contact with as a result of her compliance with this Order.
Paragraph 100 is amended to provide as follows:
100. Licensee may petition to have the restriction and conditions removed from her license at any regularly scheduled Board meeting following two years from the date of this Amended Stipulation and Consent Order, provided: (1) Licensee's supervising psychologist and treating therapist conclude in writing to the Board that Licensee is able to conduct herself in a fit, competent, and ethical manner in her practice of psychology: and (2) Licensee's petition is received by the Board at least 20 days prior to the Board meeting. At the time of Licensee's petition, she shall have the burden of proving by a preponderance of the evidence that she is capable of practicing psychology without restrictions or conditions and has complied with the restrictions and conditions above; however, Licensee's compliance with the restriction and conditions shall not create the presumption that the restriction and conditions should be removed. If Licensee's petition is denied by the Board, Licensee shall not petition for a period of one year and, if again denied, in one-year intervals thereafter. The Board may, at any regularly scheduled meeting at which Licensee has made timely petition, take any of the following actions:
a. Remove the restrictions or conditions attached to the license of Licensee;
b. Amend the restrictions or conditions attached to the license of Licensee;
c. Continue the restrictions or conditions attached to the license of Licensee upon her failure to meet her burden of proof; and
d. Impose additional disciplinary actions on the license of Licensee.
5. Except as herein amended, all terms, conditions and requirements of the attached May 7, 1999, Stipulation and Consent Order shall remain in full force and effect and are fully incorporated herein.
6. This·Amended Stipulation and Consent Order, including the attached Stipulation and Consent Order is a public document and will be sent to all appropriate data banks.

ORDER
Upon consideration of this stipulation and all the files, records, and proceedings herein, IT IS HEREBY ORDERED that the terms of this appended stipulation are adopted and implemented by the Board this _17th_ day of September___,1999
